Step 1: Download and complete both forms below

Step 2: Email the completed forms and a copy of your pet's medical records to: longpointahrecords@gmail.com with subject line "potential new client - first & last name"

Step 3: Please allow 3-5 business days for our team to review your information. A member of our reception team will contact you once your information has been reviewed.

Please note that submitting a New Client Form does not automatically establish you as a client of our practice. New client requests are reviewed based on appointment availability, current client capacity, and the review of your pet's medical records. Our team will contact you after reviewing your submission to discuss next steps.
